Thus no long-term effects of caffeine on blood pressure, heart rate, or plasma rennin activity were demonstrated. Furthermore, in the short term, Bracco et al. 17 ; did not find a significantly altered heart rate during the day after 4 mg caffeine per kg body weight was consumed 5 times daily. Accordingly, the use of caffeine is relatively safe, as it is quite certain that, although acute caffeine consumption may alter some cardiovascular variables, chronic ingestion of caffeine has little or no health consequences. In contrast, a number of safety concerns have been raised about the use of ephedrine and ephedrine-containing mixtures. In an FDA-sponsored analysis, Haller and Benowitz 50 ; reviewed 140 reports of adverse events related to the use of dietary supplements containing ephedra alkaloids. They concluded that the use of ephedra alkaloids may pose a health risk to some persons, since ephedra and related alkaloids have been associated with adverse cardiovascular events 50 ; . Haller et al. 51 ; found that consumption of two doses 5 h apart ; of an ephedra caffeine mixture 325 90 mg ; resulted in persistent increases in heart rate, blood pressure, and glucose concentrations, and decreases in potassium concentrations. Further evidence was found in a meta-analysis by Shekelle et al. 106 ; to conclude that ephedrine and ephedra are associated with increased risk of psychiatric symptoms, autonomic symptoms, upper gastrointestinal symptoms, and heart palpitations 106 ; . Given the frequency of reports of adverse cardiovascular effects, the FDA has banned the sale of ephedra-containing dietary supplements 91 ; . Other research however, observed no significant adverse effects or only temporary minor adverse effects with the consumption of ephedra alkaloids 5, 13, 14, ; . Given these contrary results, consideration should be given to whether these adverse effects occur at a higher rate than that of a matched, untreated group 13 ; . Thus the use of products containing ephedra alkaloids remains controversial and warrants further research. As mentioned previously, the long-term use of capsaicin may be limited by its strong pungency. An alternative is using CH-19 Sweet, the nonpungent cultivar of red pepper 82 ; . Green tea has been widely consumed in China and Japan for many centuries and is regarded as safe. A possible side effect of green tea consumption is a minor increase in blood pressure as seen by Berube-Parent et al. 11 ; . They observed a nonsignificant increase 7 mmHg ; in 24 h systolic blood pressure accompanied by a significant increase 5 mmHg ; in 24-h diastolic blood pressure. No increase in heart rate was seen 11 ; . This small short-term increase in blood pressure induced by green tea might be neglected since systolic blood pressure, diastolic blood pressure, and heart rate were not affected by green tea in other short-term 32 ; or long-term research 22, 27 ; . In this regard, ultraviolet light C UV-C ; irradiation has been demonstrated to inactivate both enveloped and non enveloped viruses. In this study, we examined the UV-C dose response effect delivered by a continuous-flow UV-C reactor on purified immunoglobulin IgG ; solutions. The UV-C irradiated solutions were characterised for protein composition by SE-HPLC. In parallel, pathogen inactivation using encephalomyocarditis virus EMCV ; and minute virus of mice MVM ; was examined. UV-C fluencies were identified which ensure virus inactivation while maintaining immunoglobulin integrity. Affinity chromatography with a Protein G column was used to deplete samples of intact IgG from untreated, pasteurised, and UV-C treated IgG. The non-binding fraction was analysed by 2D-PAGE, allowing the level of degraded IgG to be determined. The 2D-PAGE patterns showed that UV-C treated samples contained less degraded protein. A stability trial monitoring the functional integrity and protein compositional integrity of the final product demonstrated that UV-C treated samples of IgG maintained integrity over nine months, with results comparable to pasteurised final product. These studies demonstrated that UV-C irradiation can be effectively used to inactivate viruses, while maintaining acceptable protein and functional integrity of IgG in both the short to mid term. This work has demonstrated that UV-C treatment of IgG is a viable viral inactivation technique for therapeutic proteins.

RA ; . Early trials showed their toxicity while recent trials suggest superior efficacy. Trials of DMARD combinations have enrolled different types of patient early or established RA ; , used different designs step-up, parallel or step-down ; and utilized a range of outcome measures. We undertook a systematic review of combination DMARD therapy for RA and carried out a meta-analysis to evaluate the evidence for efficacy and toxicity. Method. Medline, PubMed and EmBase were searched using MESH headlines 'arthritis, rheumatoid', 'drug therapy, combination' and 'randomized controlled trial' RCT ; for papers published from 1975 to April 2004. References from published articles were also searched. Three independent assessors evaluated abstracts and selected trials for detailed examination. Trials were excluded if their quality was poor, were not published in English or studied DMARDs not licensed to treat RA. Two independent assessors extracted data. Efficacy was assessed by the numbers of patients withdrawn due to lack of efficacy. Toxicity was assessed by the numbers of patients withdrawn due to adverse events. Risk ratios RR ; with 95% confidence intervals CI ; were calculated and meta-analysis was carried out based on a random effects model. Sensitivity analyses evaluated different treatment combinations, trial designs, study populations and outcome measures. Results. Fifty-three potentially relevant RCTs were identified. Twelve were excluded due to: using unlicensed DMARDs n 3 reporting in journal supplements of RCTs already included n 2 follow-up of an earlier RCT, report of biological outcomes or pharmacokinetics n 5 and non-English language publications n 2 ; . Forty-one RCTs were evaluated in detail and another five excluded three open-labelled studies and two with high patient attrition 36 studies were included in the meta-analysis. These comprised 13 step-up, 16 parallel and 7 step-down trials. Nine assessed early RA and 27 established RA. Seven added steroids to DMARD monotherapy and one study added steroids to DMARD combinations. Six assessed methotrexate MTX ; plus tumour necrosis factor TNF ; inhibitors. Overall, combination DMARD therapy was more effective than monotherapy RR 0.35; 95% CI 0.28, 0.45 ; although the risk of toxicity was also slightly higher RR 1.37; 95% CI 1.16, 1.62 ; . Combinations of MTX with TNF inhibitors and MTX with sulphasalazine or anti-malarials showed good efficacy toxicity ratios. Conclusions. DMARD combinations vary in their efficacy toxicity ratio. Young was wrong about the form of government intervention which would produce the meritocracy, for he predicted that grammar school teachers would be paid similar salaries to industrial scientists as a result of legislation! He didn't see that market forces would be used as the engine of change. But he was right in predicting that education would be used to create a meritocratic elite and that egalitarian approaches to education would wane. It did not matter much about the defective, maladjusted and delinquent . It did not matter so much about the secondary modern schools. In an ideal world, not hampered by shortage of resources, the unfortunate could have large sums spent on them too. But it was not, has not been, nor ever will be, an ideal world. The choice was between priorities, and there was no doubt how the decision had to go. What mattered most were primary schools, where the pupils were being divided into the gifted and the ungifted: and, above all, the grammar schools where the gifted received their due. Thus, the biological significance of such binding would be the lack of adverse effects from this MT-bound fraction of cadmium, a notion that has value for interpreting responses of individuals in metalcontaminated environments reviewed in Roesijadi, 1992 ; . Our findings of a strong inverse relationship between MT expression and the toxicity of cadmium will need to be extended to an understanding of how MT is expressed and functions in natural populations. Our study showed that, of the metals examined, cadmium was the most effective in inducing MT in isolated oyster hemocytes and that expression of MT conferred protection against cadmium toxicity. The latter conclusion was deduced from a loss-of-function experiment in which cadmium concentrations normally not cytotoxic became highly toxic after MT expression was disrupted with antisense oligodeoxynucleotides. Thus, in molluscs, as in higher animals, MT protects against cadmium toxicity. However, the finding that MT is important in protection against cadmium does not preclude the possibility that MT is also involved in other important biological functions and alfuzosin.
