NitroMed is a research-based pharmaceutical company. Its first drug to market, BiDil, was approved this year in the United States for black patients diagnosed with heart failure. African Americans are 2.5 times more likely than the general population in the USA to die prematurely from heart failure. BiDil was initially rejected by the US's Food and Drug Administration before being shown by NitroMed to be especially useful in treating black patients. NitroMed is researching why BiDil provides a disproportionate clinical benefit to black patients, and is beginning to isolate specific bio-markers that can more precisely identify for whom the drug is most likely to be effective. The company is currently developing "second generation" derivatives as well as other complimentary products. Why the company is a pioneer BiDil may encourage an increased focus on the delivery of "personalised medicines" and the healthcare needs of minority populations. This market is at present underserved by large pharmaceutical companies, but is extensive: in cash terms, the medical needs of black patients in the US are greater than the entire German pharmaceutical market. BiDil may also encourage a different approach to pharmaceutical research, with more racially diverse drug testing and bioflavonoids.

CONCLUSION Ethnic genetic tests are highly undesirable and are on their way to widespread use in the United States. The first has already been granted in Europe. The existence of racetargeted patented drugs like BiDil in the U.S. market is a strong indication that racially or ethnically based medical techniques are now in the process of gaining acceptability in the United States. Race-based drugs are a small step compared to the problems involved in allowing ethnic genetic testing, which has deeper, more insidious, and more widespread negative effects. The use and acceptance of ethnic genetic tests can be averted, but that demands action. Existing anti-discrimination legislation provides one avenue to prevent these tests, but that response is uncertain, slow, and reactive. More useful as a prospective tool is the possibility of enacting narrow legislation specifically tailored to avoid these tests. A 4-month-old male Bernese mountain dog presented with a three-week history of progressive tetraparesis, tremors particularly of the hind limbs and depressed mentation. The dog had previously been treated with corticosteroids, but the condition deteriorated rapidly. At the request of the owner, the dog was euthanatized and a complete necropsy was performed. On gross examination of the brain there was a moderate enlargement of the lateral ventricles. Histological changes consisted of a marked proliferation of astrocytes with abnormally large cell bodies, and of numerous eosinophilic, round or elongated bodies distributed throughout the central nervous system, but most pronounced in the brain stem and midbrain. These deposits were identified as Rosenthal fibres, as they were strongly immunoreactive with antibodies to GFAP and ubiquitin, but not to vimentin; at the ultrastructural level, they consisted of electron-dense amorphous masses located within the processes of astrocytes, most frequently in the perivascular astrocyte feet, and they were tightly associated with dense aggregates of glial filaments. The histological and immunohistochemical findings of this canine case were consistent with the published neuropathological descriptions of Alexander's disease in human beings, a rare condition caused by dominant mutations in the GFAP gene and bilberry.
