Background: Current vaccines need refrigeration. However, twelve vaccines have been stabilised by drying in a sugar glass but these then require rehydration. What is needed is a completely stable liquid vaccine that requires no refrigeration and that can be injected without any preparation. Methods: Tetanus vaccine was spray dried as sugar-glass microspheres; the powder was then suspended in an anhydrous, injectable, liquid perfluorocarbon PFC ; . PFCs have advantages over oils. They do not oxidise or become rancid, are chemically non reactive, and have high density, low viscosity and low surface tension that makes them ideal to inject either via a needle or a jet injector. Their extreme lack of toxicity also makes them ideal for vaccine delivery. The glass microspheres were ballasted with calcium phosphate to exactly match the density of the PFC liquid so they had neutral buoyancy and the suspension was physically stable. Results: Four model biologicals stabilised in PFC suspensions; EPO, GMCSF, porcine somatotrophin and alkaline phosphatase showed no degradation at 50C. Tetanus vaccine in PFCs in pre-clinical trials at NIBSC, before and after storage at 37C for one month, showed no adverse reactions and all guinea pigs injected with the PFC based vaccine developed protective antibody levels equivalent to those in the water-based vaccine controls. Conclusions: Most if not all vaccines are suitable for sugar-glass stabilisation and then formulating as stable suspensions in PFCs, which can be stored at any ambient temperature. They can be pre-packaged on the shelf in ready-to-inject single dose devices.
Than at the ends of the glycocalyx, the anionic sites may be either on the sides of the filamentous projections or at the surface of the membrane. This is in contrast to the rather loose arrangement of PCF binding to the membrane around the transition region of the cilium. We think that in this region the anionic sites are associated with the five or six surface ridges that encircle the membrane even though PCF did not bind to each set of ridges on any one cilium. The number of membrane surface ridges around the transition region corresponds to the number of rows of intramembrane particles in the ciliary necklace. Furthermore, the distance between ridges 245 ]k ; agrees with the distance between strands 250 A ; in mammalian cilia 11 ; . It not unlikely that the integral proteins of the necklack contribute to the structure of the ridges. This seems to be true for mollusk ciliary membranes 9 ; . If this is the case, then these proteins, represented by intramembrane particles in freezecleaved preparations, may contain the anionic sites. The random binding of PCF to the membrane of the main part of the cilium may also correspond to anionic sites associated with the scattered intramembrane particles that have been noted in this part of the membrane 11 ; . The distribution of negative charges on the surfaces of several types of cells 2, 13, 17, ; as well as on mitochondrial 14 ; and chromaffin granule 6 ; membranes has been described. Usually, the charges are distributed evenly on these membranes; however, in human fibroblasts there are discrete regions of charge separated by segments of membrane that apparently are free of any negative charge 12 ; . In both human fibroblasts1 and baby hamster kidney BHK ; cells 13 ; , bound PCF is cleared from the surface by a process that involves movement of ferritin into endocytic vesicles. Although in our experiment this process was observed in adjacent secretory cells, no movement of PCF on the surfaces of ciliary membranes was detected after 40 min incubation in PCF-free media. This corresponds to the fact that very little endocytosis of ferritin was detected in these cells. Therefore, the ciliary membrane appears to be relatively stable x Basu, S. K., R. G. W. Anderson, J. Arthritis [2-4]. Therefore, therapeutic targets have been introduced in order to neutralize the biological effects of these cytokines [5]. TNF is a pro-inflammatory cytokine that many cells, monocytes and T lymphocytes, produce in response to various stimuli, notably microorganisms. It is also involved in a wide range of physiologic processes, such as apoptosis, cell activation, induction of other inflammatory cytokines, and recruitment of inflammatory cells to sites of infection. TNF also plays a role in cancer immune surveillance [6]. Beside these physiologic roles, TNF have deleterious effects in inflammatory conditions such as RA, SpA, JIA, and also in psoriasis, Crohn's disease and a wide range of systemic diseases including sarcoidosis and polymyositis ; [7]. The knowledge of this important role led to the development of TNF antagonists [8, 9]. However, because of the central role of TNF in inflammation and particularly in the normal immune response to pathogens, the question of an infectious risk in TNF antagonist recipients is a relevant issue. Thus, infectious complications have been observed during the clinical trials of these agents and post marketing reports gave also additional data [10]. The spectrum of pathogens that have been associated to anti TNF therapies ranges from common bacteria to more opportunistic organisms such as Mycobacterium tuberculosis [11]. The infection may have a benign course or may be a serious, life threatening disease, and the extent of infection may be localized or disseminated. Moreover, it appears that the pattern of infections under TNF blockade therapy involved certain pathogens and that the different TNF blockers are not associated with the same risk of infection and clofarabine.

Standardization of the Prothrombin Time The history of standardization of the prothrombin time has been reviewed.13638 A calibration system was developed based on the linear relation between the logarithm of the prothrombin time ratios obtained with reference and test thromboplastins. This calibration model, known as the International Normalized Ratio INR ; , was adopted by the World Health Organization in 1982 and is used to standardize the reporting of prothrombin time by converting the prothrombin time ratio determined with any local thromboplastin into an INR equal to observed prothrombin time ratioC, where the prothrombin time ratio is the patient's prothrombin time divided by normal pooled plasma prothrombin time and C is the International Sensitivity Index [ISI] for each thromboplastin ; . The ISI is a measure of the responsiveness of a given thromboplastin to reduction of the vitamin K-dependent coagulation factors; the lower the ISI, the more responsive the reagent and the closer the derived INR will be to the observed prothrombin time ratio. The INR is the prothrombin time ratio that would be obtained if the WHO reference thromboplastin, which by definition has an ISI of 1.0, were used to assess prothrombin time.1'38 Examples of the conversion of prothrombin time ratios to corresponding INRs for thromboplastins with three different ISI values are shown in Table 3. It is clear that the same prothrombin time ratio can reflect markedly different levels of anticoagulation, depending on the ISI value and cilium.
