There is a difference in therapy results between two groups of AML patients: with blastic bone marrow infiltration within 20-29% and 30%. We provided retrospective analysis to answer to this question. The analysis was performed on patients population treated according to PALG Polish Adult Leukemia Group ; protocol of DAC vs. DA study. There were 26 cases with myeloblastic bone marrow infiltration within 20-29% assessed group ; , and 379 patients with infiltration 30% at diagnosis comparative group ; treated within the above mentioned study in years 1999-2002. The patients received as induction DAC-7 regimen: daunorubicin 60 mg m2 d iv 1-3; cytarabine 200 mg m2 d ci 17; cladribine 5 mg m2 2h inf. iv d 1-5 or standard DA-7 regimen the same regimen without cladribine ; . Patients achieving CR received two courses of subsequent intensive consolidation: 1 ; HAM HD AraC, mitoxantrone ; 2 ; HD AraC with or without cladribine in the DAC-7 or DA7 arm, respectively. In the case of PR after the first induction course the same regimen was repeated. Post-consolidation therapy was in both arms comparable. In 62, 5% patients n 10 ; of the assessed group AML was preceding by myelodysplastic syndrome. Complete remission CR ; rate was comparable in both populations and reached 73% n 19 ; in the assessed group and 70% n 264 ; in the comparative one. Simultaneously, there was no statistical difference between overall survival OS ; and leukemia free survival LFS ; in both groups. OS after 5 years reached 34% in the assessed group and 27% in the comparative one and LFS reached 42% and 27%, respectively p NS ; . Our retrospective study did not show any difference in therapy results between AML patients with bone marrow infiltration within 20-29% and 30% at diagnosis and it can be an additional argument to join both groups to identical therapeutic proceedings.
Ascertain whether a clone of VRE persisted in the units or whether different clones had been introduced at different times. At the same time all the strains were evaluated by infrared spectroscopy FT-IR ; and the results obtained were compared with the reference method PFGE ; . The results of PFGE allowed us to identify fourteen VRE clones and was documented a temporal shift in the prevalence of these clones. Among the fourteen clones we identified one major clone of VRE responsible for the main cases of nosocomial infection designated Sma 1. By FT-IR we identified thirteen VRE clones and were also documented a temporal shift in the prevalence of these clones. The molecular typing PFGE ; and phenotypic typing FT-IR ; showed concordant identification of the clones and the same temporal shift in their prevalence during the study period. Although more limited than PFGE in discriminatory power, infrared spectroscopy FT-IR ; could be used as a screening method for typing VRE strains because of the shorter turnaround time, easy to perform and sharing of information.

This study investigated the effects of tumour necrosis factor-a TNF ; on human granulosa cells taken from ovaries of 11 premenopausal women undergoing oophorectomy during the luteal phase of the cycle for reasons unrelated to ovarian pathology. Granulosa cells from follicles ranging from 5-10 mm diameter small ; and from 10-25 mm large ; were subjected to culture for 48 and 96 h. Granulosa cells were cultured with human FSH 1 ng ml ; , testosterone 1 |iM ; and human TNF 10 ng ml ; , each alone, and in various combinations. In granulosa cells of small follicles, FSH alone increased progesterone and cAMP accumulation and the conversion of testosterone to oestradiol. In granulosa cells of large follicles, FSH increased progesterone and cAMP accumulation but not the conversion of testosterone to oestradiol. Only in granulosa cells of small follicles did TNF significantly inhibit FSH-induced conversion of testosterone to oestradiol but it was not apparent until the second 48 h of culture and concomitantly TNF did not alter the ability of FSH to stimulate progesterone and cAMP accumulation. In granulosa cells of large follicles, TNF did not alter FSH-stimulated oestradiol, progesterone or cAMP accumulation. Interestingly, progesterone accumulation in the presence of TNF and FSH was significantly greater in granulosa cells of large follicles than in granulosa cells of small follicles. The results indicate that TNF suppresses FSH-induced oestradiol secretion in granulosa cells from small follicles and this modulatory effect of TNF appears to be independent of decreases in progesterone and cAMP. The potential physiological significance of these findings is that TNF may be a relevant cytokine in suppressing FSH-induced oestradiol secretion and follicular growth during the luteal phase of the cycle.
