Constipation is the most prevalent side effect of opioid use. Management; Effective management of constipation in patients with cancer requires the appropriate use of laxatives. Review all medications taken by the patient to consider nonessential constipating drugs. A list of meds that cause constipation is provided ; . Bulk-forming laxatives: Initial therapy for mild constipation and those with good performance status or those receiving medications that may constipate them. Needs to be taken with adequate fluid and daily dose titrated to need. Frontline treatment for patients who meet fluid intake requirements. Not to be used as single-agent therapy for patient receiving opioid analgesics Harms: Not good if patient is already constipated. Don't give to patients with advance cancer or those with esophageal strictures, ulcers, stenosis, or intestinal adhesions. Pain and bloating may result. Emollient Laxatives Docusate sodium and docusate calcium ; Require increased fluid intake to facilitate stool softening. Indicated to avoid straining, in cases of acute perianal disease and when bulkforming agents are not enough to soften stool. In combination with a stimulant laxative provides a softer stool that is easier to evacuate. Harms: Do not give stool softeners with mineral oil because it may increase the absorption of the mineral oil and increase the risk of toxicity. Osmotic saline laxatives retain fluid in the bowel to form softer stool. lactulose, sorbitol, PEG, magnesium hydroxide ; These draw water into the intestinal lumen to stimulate propulsion and motility. Harm: Larger doses of lactulose tend to result in bloating and colic. Polyethylene glycol-electrolyte solution: used for GI lavage prior to procedures. Heart rate, respiratory rate, mucous membrane color, capillary refill time and reflexes are all reliable, useful parameters for monitoring the anesthetized patient. Don't forget your medicine that you learned in dogs and cats. Don't be afraid to apply the principles to your exotic patients and doxil.
There is quite a burgeoning literature on Bartonella. However, I weary of this unexamined philosophy of care that monotherapy, devised for use in the context where a person is sick with ONE AGENT, is necessarily going to work in the context of TBD [tick-borne disease] patients who are likely infected with multiple agents and who suffer from varying degrees of immunocompromise. Monotherapy, we should keep in mind, has been touted in publications in the context of those lucky souls who are infected with a "mono-agent" -- ONE agent. So, if all you have is Bartonella, chances are that maybe one agent might help get you back on track. However, there are experts who would disagree that monotherapy will work even in those cases once Bartonella becomes symptomatic -- they believe that you will need TWO AGENTS. From my review of the literature, there are few papers on the successful use of Cipro, and again, the drugs were not used in the context of Lyme disease patients coinfected with multiple agents. There is no standard of care for Bartonellosis. Like many of these therapies that Lyme patients receive all the while being told that they will be "cured, " based on what I have read in the literature -- and I have read extensively -- it seems to me another potentially "suppressive only" regimen. Bartonella is also NOT just Bartonella henselae. One patient told me she just ran out and got tested for Bartonella. What SPECIES of Bartonella, please? The abstract of Chang et al from the tick study done by the UC Davis group in Santa Clara Number 35 County states: "In these infected ticks, molecular analysis showed a VARIETY of Bartonella strains, which were closely related to a cattle Bartonella strain and to several known human-pathogenic Bartonella species and subspecies: Bartonella henselae, B. quintana, B. washoensis, and B. vinsonii subsp. berkhoffii." A negative result on a test for Bartonella henselae does not rule out Bartonella, just like a negative result on a test for Babesia microti does not rule out Babesia WA1, and a negative test for HGE does not rule out HME. A negative test on a serology for Bartonella henselae and Bartonella quintana the test available through Focus Technologies, Cypress, CA - formerly MRL ; also DOES NOT rule out other Bartonella species for which testing is not yet available through commercial venues. Ditto for Babesia and Borrelia. Theoretically, one could be merely picking up the tail of the whale in finding positive B. henselae and or quintana titers, and a person might have even higher titers to a species that has yet to be characterized in terms of its pathology in humans. That is the pattern you can see with all of these pathogens that are crossreactive--that a person may have a low reactive titer in comparison to the titer to the organism they appear to be truly infected with. So, for example, a low positive titer to B. henselae could indicate an infection with B quintana that has not been tested for, or another species which is presently impossible to test for commercially. One theoretical way to get around and docusate.

Complete occlusion of the ear canal by cerumen is a frequent cause of conductive hearing loss. Cotton-tipped applicators are notorious for worsening cerumen impaction. Warm water body temperature ; irrigation is a safe method of removing cerumen in patients who have no history of otitis media, perforation of the tympanic membrane, or otologic surgery. Use of an otoscope and a curette allows cerumen to be removed under direct vision. The distance to the tympanic membrane must be kept in mind, because otoscopes do not allow for depth perception. A variety of softening preparations are available if cerumen is too firm to remove. Aqueous-based preparations, including docusate sodium, sodium bicarbonate, and hydrogen peroxide, are effective cerumenolytics.2, 3 Foreign bodies in the external auditory canal also can cause unilateral conductive hearing loss. These foreign bodies can be removed by irrigation or with a curette. If the object is not impacted or hygrostatic, warm water irrigation probably should be attempted first. If this approach is not effective, the foreign body can be removed with an instrument if the patient is cooperative. If the patient is uncooperative, removal in an operating room may be necessary.
