Chemical Structure of Fenugreek Gum Legume seed galactomannans generally consists of a , 1 -linked linear mannan backbone, to which single galactose grafts are linked randomly by an , 1 glycoside bond. Fenugreek galactomannan has this common structural feature. Galactomannans from different legume seeds differ in their M: G ratio, molecular weight and mode of placement of the galactose grafts, which are generally not spaced regularly but placed randomly and in blocks on the linear backbone. Fenugreek galactomannan is unique in having mannose and galactose in nearly 1: ratio, and it is more cold water-soluble than any other galactomannans including guar gum. Since the M: G ratio in fenugreek gum is close to 1; it makes FG-gum as one of the highest galactose ~ 48%; M: G, 1.02: 1 ; containing galactomannan. Besides fenugreek, Lucerne Medicago sativa ; and clover Trifolium pratense ; are two other, though less common galactomannans, which have ~48 percent galactose. The linear mannan backbone of FG-gum thus have 1 6 ; linked single galactose grafts on nearly all the mannose groups of the main chain. Molecular weight of FG-gum is ~30, 000 Daltons, corresponding to an average presence of 180-190 monosaccharide mannose + galactose ; units in a molecule. In practice, isolates would have to be tested individually to guide therapy. It is worth noting that clarithromycin-resistant mutants have a unique pattern of acquired cross-resistance to macrolides and related antibiotics, but do not differ from clarithromycin-sensitive strains in terms of resistance to non-macrolide antibiotics.21 There have been few studies of the activities of these compounds in vivo. Klemens & Cynamon22 looked at the activities of clarithromycin and azithromycin against a variety of nontuberculous mycobacteria in a beige mouse model. They found that both macrolides had activities comparable to, or better than, those of conventional agents such as rifampicin, ethambutol or clofazimine in the test system. There are no systematic clinical trials of these agents. Reviewing the literature, Rapp et al.14 noted several anecdotal reports of the use of new macrolides in treatment, either alone or in combination with other agents, with variable responses. Their biosynthesis in developing legume seeds involves the functional interaction of two membrane-bound glycosyltransferases, mannan synthase MS ; and galactomannan galactosyltransferase GMGT ; . MS catalyzes the elongation of the mannan backbone, whereas GMGT action determines the distribution and amount of Gal substitution. Fenugreek Trigonella foenum-graecum ; forms a galactomannan with a very high degree of Gal substitution Man Gal 1.1 ; , and its GMGT has been characterized. We now report that the endosperm cell walls of the tobacco Nicotiana tabacum ; seed are rich in a galactomannan with a very low degree of Gal substitution Man Gal about 20 ; and that its depositional time course is closely correlated with membrane-bound MS and GMGT activities. Furthermore, we demonstrate that seeds from transgenic tobacco lines that express fenugreek GMGT constitutively in membrane-bound form have endosperm galactomannans with increased average degrees of Gal substitution Man Gal about 10 in T1 generation seeds and about 7.5 in T2 generation seeds ; . Membrane-bound enzyme systems from transgenic seed endosperms form galactomannans in vitro that are more highly Gal substituted than those formed by controls under identical conditions. Testing of 8R- and 8S-terguride, the dihydro-analogs of 8R- and 8S-lisuride, respectively, and a variety of other derivatives revealed that 8R-lisuride is by far the most potent H1R agonist identified in this study Fig. 5 and Table 1 ; . These data indicate that the molecular structure of 8R-lisuride is optimal for H1R activation compared with the other tested compounds, which is in agreement with binding data reported in literature for rat or guinea pig H1Rs Beart et al., 1986; Millan et al., 2002 ; . Health benefit of fenugreek
I felt reasonably comfortable to consider my own diagnosis or interpretation and at times challenged the label given by another health worker. For example, I asked one female Burmese health worker who was removing foetal tissue with sponge forceps from a haemorrhaging woman's uterus without analgesia, if her patients ever experienced shock in the medical sense of an increasing pulse rate and respirations, drop in blood pressure, confusion ; during these perfunctory D&Cs. She replied she never had this problem. The patient was clearly not only in pain but also clinical shock, indicated by her pale, sweating face, cold extremities and fast respirations. I offered to take her blood pressure for the health worker while she continued and when we both noted the falling pressure, she asked me to increase the saline drip. We did not need to discuss it further. In another case, during a D&C where I was the torch holder, the health worker brushed her sponge forceps into the patient's bowel motions and was about to reintroduce the sponge forceps into the uterus, an easy mistake to make where there is no overhead light-source, nor lithotomy couch to position the patient for this special procedure. I intervened and helped her get fresh clean ones to continue but I wonder if non-medically trained researchers would have noticed this detail? Some may argue that I was too interventionist but these were also ethical dilemmas how could I have not intervened, and simply observed a patient sinking into preventable shock or experiencing a painful pelvic infection that could potentially damage her future fertility? I also found it useful to be able to offer some competent contraceptive advice to women who agreed to be interviewed, and who through the course of the interview, demonstrated their lack of knowledge of modern contraceptives and a desire to limit their fertility. But medicine's hegemonic ontology can be a block and must not obstruct the anthropological imagination. I both midwife and researcher and my research reflects both of these passions. To straddle both disciplines and work at the margin can be insightful, as Kleinman 1995: 3 ; reflects.

In response to Mr. Bob Morrow's letter of March 29, 1998, Mr. Archibald confirmed that the CWF supported the Applicant's request for reinstatement on the grounds that the penalty was not consistent with the International Weightlifting Federation and International Olympic Committee sanctions for stimulant offences. The gist of Mr. Archibald's evidence is that stimulants should not be treated in the same way as steroids and that for a first "Sudafed" offence a four month suspension would be appropriate and fenugreek.

Moreover, the only patient in our series of unilateral subthalamotomy with severe hemiballism suffered a large infarction of the region Obeso et al., 2001b no other patient with unilateral subthalamotomy had a lesion as large as the ones associated with hemiballism in the group with bilateral subthalamotomy. Interestingly, lesions clearly spreading dorsally and reaching the thalamus have been encountered in parkinsonian patients who developed severe hemiballism Dierssen et al., 1961; Obeso et al., 2001b; Tseng et al., 2003 ; . It appears, therefore, that the larger the lesion the greater the volume of the STN affected. but also the probability of destroying the pallidothalamic fibres running dorsally Baron et al., 2001; Hamani et al., 2003 ; . Thus, we find it difficult to reconcile our observations with the view that subthalamotomy has a pallidotomy-like effect due to dorsal extension of the lesion to interrupt the thalamic fasciculus Lozano, 2001; Su et al., 2001; Tseng et al., 2003 ; . It may be that large lesions affect other subcortical regions that are also relevant for the induction of dyskinesias. For example, blockade of the zona incerta with bicuculline in the rat has been shown to induce involuntary movements similar to those provoked by STN inhibition Perier et al., 2002 ; . The effect of simultaneous bilateral lesion of the STN may be another concurrent variable. The data reported here may be taken to suggest that bilateral lesions performed simultaneously are associated with a higher incidence of severe dyskinesias. However, both patients populations i.e. staged versus simultaneous surgery ; were not really identical in terms of the severity of dyskinesias preoperatively and overall exposure to levodopa. Thus, surgery in the initial patients included in this study was staged and we purposely chose subjects who had not developed severe LIDs. Overall, the experience is limited and the number of intervening variables i.e. patients' preoperative state, dose of levodopa prior to surgery, topography and size of the lesion, etc. ; too large to allow an accurate assessment of the effect of bilateral and simultaneous lesion and dyskinesias. Meanwhile, a cautious approach would be to defer subthalamotomy in patients with severe LIDs and consider pallidotomy instead Scott et al., 2002 ; .
