Although the use of cannabinoids for nausea associated with cancer chemotherapy is established, a variety of other uses have been suggested over the years. The drug has some effects on the canal that drains the eye, and is thus useful in increasing the drainage in patients with glaucoma, a condition of elevated intra-ocular pressure that can cause blindness. This effect of marijuana is not in dispute, although there are other drugs that have a similar effect. The problem is that the effects are transient, and thus, either the pressure would again increase or the individual would require additional marijuana. One estimate suggests that, to provide adequate control of intra-ocular pressure with marijuana, an individual would have to smoke about 4, 000 joints a year. Although this observation has been disputed, certainly cannabis has a rather short duration of action in terms of its effects on mood, and it would not be surprising to find that the effects on intra-ocular pressure were similarly shortlived. This suggests, however, that synthetic derivatives, perhaps with a reduced tendency to metabolism or storage in fat, might prove useful, and it is clear that the American Academy of Ophthalmology is supportive of further study. It has been demonstrated that activation of the cannabinoid receptor does lower intra-ocular pressure, but another small study suggested that tolerance developed to the effects and all the patients in the study chose to discontinue therapy with cannabis. There have been no further advances in this area. A use of marijuana for which there is a massive amount of anecdotal information, but fewer clinical studies, is in appetite promotion. Users of marijuana have reported for years that it increases appetite, and there are a number of conditions in which this property would be desirable. Terminal conditions including advanced cancer and AIDS are often accompanied by cachexia, or "wasting, " in which all interest in food vanishes. An agent that stimulates appetite would be welcome in this group of patients. There is a significant amount of evidence that body weight is maintained and appetite increased in patients with cachexia who are treated with cannabis, and one article describes this effect as "proven."22 Despite this, there have been no large.

The utilization of IL7R as a possible drug target for MS treatment is a difficult issue. The unique properties of the receptor, being a non-redundant cytokine receptor make modulation of its biological function challenging. In human immunodeficiency virus HIV ; infection the possibility of targeting the IL7 IL7R complex for therapy has been discussed, but the situation in HIV is somewhat different from MS. Their main concern regarding using IL7 therapeutics for HIV is the risk of increased viral infectivity and the risk of autoimmunity reviewed in [206] ; . Both these concerns would be even more problematic in MS, due to the autoimmune nature of the disease and the proposed influence of virus infections in MS pathogenesis. Using IL7 IL7R as a drug target in MS would need further biological understanding of this complex and the signalling pathways in order to modulate this system in a beneficial manner for MS patients. Even though the genetic data support an important role of IL7R in MS, the exact biological and functional consequences of the genetic findings are still to be elucidated. A genetic association could be regarded as a statistical finding until the biological effect of the genetic variant has been demonstrated and understood. In general, the association of IL7R strengthens the hypothesis of MS as autoimmune and T-cell mediated disease. It also gives credit to the disease modifying treatments used as of today in MS care, since the present treatments all have mechanisms of action developed on the basis of MS as autoimmune and T-cell mediated disease.

Abstract Chemotherapy-induced nausea and vomiting CINV ; remains a significant problem in the care of cancer patients. Although the use of serotonin 5-HT3 ; receptor antagonists, as well as neurokinin-1 inhibitors, has reduced rates of acute emesis, many patients still experience acute vomiting; moreover, these agents have reduced efficacy in preventing nausea, delayed CINV, and breakthrough CINV. Nausea, in particular, continues to have a major--and often overlooked--impact on patients' quality of life. Optimizing the treatment for CINV likely will involve combinations of agents that inhibit the numerous neurotransmitter systems involved in nausea and vomiting reflexes. Cannabinoids are active in many of these systems, and two oral formulations, dronabinol Marinol ; and nabilone Cesamet ; , are approved by the US Food and Drug Administration for use in CINV refractory to conventional antiemetic therapy. Agents in this class have shown superiority to dopamine receptor antagonists in preventing CINV, and there is some evidence that the combination of a dopamine antagonist and cannabinoid is superior to either alone and is particularly effective in preventing nausea. The presence of side effects from the cannabinoids may have slowed their adoption into clinical practice, but in a number of comparative clinical trials, patients have expressed a clear preference for the cannabinoid, choosing its efficacy over any undesired effects. Improvement in antiemetic therapy across the entire spectrum of CINV will involve the use of agents with different mechanisms of action in concurrent or sequential combinations, and the best such combinations should be identified. In this effort, the utility of the cannabinoids should not be overlooked and meclizine.

4. Discussion 4.1. Species composition and family level differences This study has provided a rare comparison of targeted fish assemblages in sanctuary and fished areas, where recreational line fishing is the only pressure exerted on fish stocks. Under these circumstances, we have shown that the sanctuary zones studied at Ningaloo Marine Park support a different composition of targeted fish families genera to adjacent recreation zones. Differences in fish composition between fished and unfished zones have been demonstrated in other coral reef MPA comparisons Jennings et al., 1996b; Rakitin and Kramer, 1996; Roberts and Hawkins, 1997; Wantiez et al., 1997; Chiappone and Sealey, 2000 ; , but where fishing pressure was exerted by commercial or artisanal fishers. Our study has shown that recreational line fishing alone may, in some cases, be sufficient to alter the composition of targeted fish. Trends for fish family genera composition in terms of biomass were stronger than those using abundance data. Mandu showed the greatest difference between zones of the three regions. Although not significant, there was a separation in nMDS ordinations between zones for biomass at Osprey and Maud. Differences in assemblages were driven by lethrinids, which constituted approximately 78% of all fish censused and differed between zones in terms of biomass, mean size, and abundance of legal-sized fish. We recorded no differences in other families genera Lutjanidae, Haemulidae, Serranidae, and Choerodon spp. ; between zones. However, these families combined formed less than 22% of fish censused. The mean size of lethrinids was smaller in the fished zones, which may be the result of recreational fishers removing the larger legal-sized fish. This was supported by the fact that legal-sized lethrinids were significantly more abundant in the sanctuary zones i.e., where there was no fishing pressure ; .
