Lands is not discussed in the same level of detail as is the discussion of sites north of the Deep Water Ship Channel. It is estimated that approximately 50-150 additional housing units will be constructed by mid-1996. The majority of these units will be single family homes in the Windsor Meadows subdivision located west of Jefferson Boulevard and south of South Linden Road. Building permits were taken out on some of these lots in 1993 but were never finaled. An additional series of single family homes could be constructed in the Linden West subdivision which is located east of Jefferson Boulevard and south of Linden Road. Should a bridge be constructed over the Deep Water Ship Channel several hundred additional housing units could be constructed by mid-1998. Availability of Infrastructure and Off-Site Improvements for Vacant Lands South of the Deep Water Ship Channel There is limited infrastructure within the Southport area which will affect large scale development through 1996. These infrastructure shortfalls and environmental constraints include the need for additional transportation improvements construction of an additional bridge ; , adequate drainage plans, provision of adequate water and wastewater treatment. Swainson's hawk mitigation is discussed later in this chapter under Local Land Use Regulations. Transportation Development is constrained in the Southport area in the short term because of poor access; the area is currently served by a single two-lane bridge on Jefferson Boulevard across the barge canal. In July, 1990 the City determined that the Jefferson Boulevard bridge which provides the only access from West Sacramento to Southport, is at capacity. The Jefferson Boulevard bridge is a bascule bridge over a barge canal and is crossed immediately north of the bridge by the rail line accessing the Port of Sacramento. When the bridge is up or train is crossing, Southport is effectively isolated from normal and emergency service traffic. In August, 1990, the West Sacramento Planning Commission indicated that until at least the Industrial Boulevard bridge or expansion of the Jefferson Boulevard bridge is completed, new development in Southport will be limited to construction on existing Assessor's parcels of record as of August 16, 1990. This action, however, allowed the following exceptions: subdivisions on fully built-out parcels, neighborhood serving commercial uses on a case-by-case basis, development of one single family unit per lot on subdivisions accepted for review prior to July 1990, and industrial development under a Transportation Management Plan. Planning actions such as tentative subdivision and parcel maps are still being processed under this policy but physical construction of new units will not be allowed until a new or expanded bridge is open.
Clinical Pearl: For patients whose nutrition is being monitored closely, an increase in the prealbumin of 1 mg day indicates a good response to nutritional supplementation. A response of 2 mg week signifies an inadequate response or an inadequate level of nutritional support.

Patients with IPAH demonstrated the greatest increase in 6MWD, although haemodynamics remained unchanged. Moreover, follow-up data showed that clinical benefits apparent after 12 weeks were not sustained at 12 months, suggesting tolerance to therapy and a lack of mortality benefit.14 Endothelin Antagonists Endothelin-1 ET-1 ; is a potent endogenous vasoconstrictor and smooth muscle mitogen primarily produced by vascular endothelial cells. Overexpression of ET-1 has been demonstrated in the plasma and lungs of patients with PAH, 16 and higher levels of ET-1 correlate with disease severity. The effects of ET-1 are mediated through activation of two endothelin receptor isoforms, termed ET-A and ET-B, with the latter predominating by approximately 2: 1.17 The deleterious effects of ET-1 appear to be mediated through activation not only of the ET-A receptor, but also ET-B receptors located on smooth muscle cells and fibroblasts. The approval of the non-selective ET-A ET-B receptor antagonist, bosentan, represented a significant breakthrough in the search for more acceptable therapeutic options for PAH, being the first effective orally-active agent available for use in the disease. An initial randomised trial demonstrated improvements in 6MWD in bosentan-treated patients, while no change was observed in those receiving placebo and penicillamine.

They are successfully used in the treatment of attention- deficit hyperactivity disorder ADHD ; . ADHD is a disorder usually diagnosed in early childhood in which the child exhibits such symptoms as short attention span, excessive activity, and impulsivity. A child with ADHD should take a stimulant medication only on the advice and under the careful supervision of a physician. The use with children of the medications described in this information packet is more limited than with adults. The total reported manufacture of the other stimulants in Schedule IV benzfetamine, phendimetrazine and pipradrol ; amounted to less than 30 million DDD in 1999 see figure 13 ; . 75. Fenproporex, a substance mainly used as an anorectic, was brought under international control in 1986. Since then, only Brazil, France and Switzerland have reported its manufacture. In the period 1995-1999, the manufacture of fenproporex in France varied greatly, averaging around 1.3 tons annually, a significant drop from the annual average of nearly 3 tons of that substance manufactured in that country in the period 1992-1994. In 1997, Switzerland reported for the first time the manufacture of fenproporex 747 kg ; . The manufacture of that substance in Switzerland increased to 1.8 tons in 1998 and 3.8 tons in 1999. Pemoline Cylert ; NOT A FIRST CHOICE FOR THE MANAGEMENT OF AD HD SYMPTOMS DUE TO POTENTIAL FOR VERY SERIOUS LIVER DAMAGE. ; Form: Long-acting tablets administered by mouth. Pemoline 18.75 mg, 37.5 mg, 75 mg. Dosage: Very individual. Duration of Action: Slow onset of action. Generally lasts 8-10 hours. Possible Side Effects: Same as methylphenidate. Effect on liver functioning of concern, but significant complications are extremely rare. There is no evidence that monitoring liver functions has predictive value with regards to averting complications; nonetheless, very frequent lab testing is recommended. Imipramine and Desipramine Tofranil and Norpramin ; Form: Tablets administered by mouth. 10 mg, 25 mg, 50 mg and 100 mg. Dosage: Very individual. Duration of Action: Variable. Often has 24-hour effect. Effect: Lower doses may improve AD HD symptoms within several days, but may take 1-3 weeks for full effect. Higher doses may improve depressive symptoms and mood swings. Possible Side Effects: Nervousness, sleep problems, fatigue, stomach upset, dizziness, dry mouth, accelerated heart rate. May affect conduction time of the heart, leading to irregular heart rate. In rare cases, may affect blood count. Should not be abruptly discontinued. Buproprion Wellbutrin ; Form: Tablets 75 mg and 100 mg; extended release as 100 mg and 150 mg. Dosage: Very individual. Duration of Action: About 4-6 hours in short-acting form; 6-8 hours in long-acting form. Effect: Improves symptoms of AD HD and can affect depressive moods. Possible Side Effects: Difficulty sleeping, headache. Clonidine Catapres ; Form: Clonidine is available in patches applied to back of shoulder or tablets administered by mouth - 0.1 mg, 0.2 mg, and 0.3 mg. Dosage: Very individual. The clonidine patch is available in three strengths. Duration of Action: Patches last 5-6 days. Tablets last 4-6 hours. Effect: Often will improve excessive hyperactivity or insomnia associated with AD HD, but has not been demonstrated effective for improving inattention symptoms. May decreases facial and vocal tics in Tourette's syndrome. Often has positive side effect on oppositional defiant behavior and may be beneficial for management of excessive anger. Possible Side Effects: Major side effect is fatigue, though this will usually disappear over time. Other side effects may include dizziness, dry mouth, increased activity, irritability, and or behavior problems. Physician should be consulted prior to discontinuation of medication to prevent "rebound hypertension" or other effects. Guanfacine Tenex and pemoline.
