The Board has increased the number of hours that a pharmacy intern may earn in a college-based clinical program from 1, 000 to 1, 250. As a result, only 250 hours rather than 500 hours ; must now be obtained apart from the college experience. In addition, the Board has lifted its restriction on "concurrent hours." As a result, pharmacy interns may now earn up to 10 hours of internship experience per week while attending college classes. The 250 hours must be acquired under the supervision of one or more preceptors in a traditional licensed general community ; or hospital pharmacy at a rate of no more than 48 hours per week. Preceptors must be licensed as a pharmacist in good standing in the state where the internship is served. Formulary update, from page 1 Pneumocystis jiroveci formerly Pneumocystis carinii ; pneumonia PJP ; . It was evaluated for formulary addition because, as an anti-infective, it is a high-priority nonformulary drug. Sulfamethoxazole-trimethoprim co-trimoxazole ; is the first-line agent for PJP according to CDC guidelines for the treatment and prevention of opportunistic infections in HIV infected adults and adolescents. It is inexpensive and effective. The CDC guidelines recommend the use of atovaquone when patients cannot tolerate co-trimoxazole. Atovaquone has the advantage of being available as an oral suspension, which is useful when patients cannot swallow solid oral dosage forms eg, patients with feeding tubes ; . Atovaquone is considerably more expensive than dapsone, but it is often preferred due to tolerability based on a more favorable adverse effect profile. Gastrointestinal adverse effects including diarrhea, constipation, nausea, vomiting, dysguesia, and dyspepsia are common with atovaquone. Although not common, hemolytic anemia is a potential adverse effect. Atovaquone suspension is an alternative to pentamidine and dapsone in patients with PJP who cannot be treated with co-trimoxazole. Truvada is a combination of 2 reverse transcriptase inhibitors, emtricitabine and tenofovir, used to treat patients with HIV. The combination decreases the pill burden in HIV patients with the goal of improving patient compliance and hopefully therapeutic response ; . The individual agents in Truvada are listed in the Formulary; however, the combination product was added to improve continuity of care for HIV patients and avoid medication errors. Revatio is a new brand of sildenafil with a labeled indication for the treatment of pulmonary arterial hypertension WHO Group I ; to improve exercise ability. Viagra was already listed in the Formulary specifically for the treatment of pulmonary hypertension. Revatio was added in the Formulary as a line extension. Revatio is available as white, 20-mg tablets that do not look like blue Viagra tablets, which come in 25-, 50-, and 100-mg tablets. The 20-mg tablets were added for patients who may be admitted on this strength. This will also prevent medication errors upon discharge, when patients could be discharged on Viagra and mistakenly resume their Revatio. There should, however, be no therapeutic difference between a 20-mg and 25-mg dose of sildena and percodan.

The antiprotozoal compound 1, 5-di 4-amidinophenoxy ; pentane pentamidine ; and 36 of its analogs were screened for in vitro activity against Leishmania mexicana amazonensis clone 669 C4S MHOM BR 73 M2269 ; and Plasmodium falciparum clones W2 Indochina HI CDC ; and D6 Sierra Leone I CDC ; . Pentamidine and each of the analogs tested exhibited activity in vitro against L. m. amazonensis and P. falciparum. The pentamidine analogs were more effective against the P. falciparum clones than against L. m. amazonensis. P. falciparum was extremely susceptible to these compounds, with 50% inhibitory concentrations as low as 0.03 FpM. While none of the analogs exhibited marked improvement in antileishmanial activity compared with pentamidine, 12 of the pentamidine analogs showed activity approximately equal to or greater than that of the parent compound. From the promising activity exhibited by the pentamidine analogs in this in vitro study and their potential for reduced toxicity relative to the parent drug, pentamidine-related compounds hold promise as new agents for the treatment of protozoal infections and pergolide.

The infection is transmitted by tsetse flies and currently occurs in 36 African countries between latitudes 14 N and 29 S, placing at least 50 million people at risk and resulting in 25, 000 new cases every year. Although West African disease is more indolent than East African disease, both infections are ultimately fatal if untreated. Drugs currently used to treat African trypanosomiasis are suramin, pentamidine, melarsoprol, eflornithine, and, experimentally, diminazene and nifurtimox. History of chemotherapy. Arsenic, a cornerstone of treatment for human trypanosomiasis affecting the CNS, was first used by the famous missionary-explorer David Livingstone to treat the analogous disease in horses nagana ; in 1847 or 1848. Arsenicals were later synthesized for human use by Ehrlich and gradually modified to decrease toxicity; by the end of World War II, melarsoprol B, a trivalent arsenical currently in use, was introduced. Suramin, a sulfated naphthylamine, and pentamidine, an aromatic diamidine, are both effective only against the bloodstream stages of T. brucei; they were introduced in 1920 and 1939, respectively. Eflornithine, the only new drug developed and approved for African trypanosomiasis in 40 years, was first used for Gambian sleeping sickness in the mid-1980s 167 ; . Mechanisms of drug action. Organic arsenicals interfere with protein sulfhydryl groups found in enzymes of carbohydrate metabolism; therefore, it is thought that the primary action of melarsoprol is to disrupt parasite glycolysis. Selectivity has been attributed to preferential binding affinity to parasite pyruvate kinase, greater dependence of the trypomastigote on glycolysis to generate ATP, and higher concentration of the drug in trypomastigotes as opposed to mammalian cells 55 ; . The antitrypanosomal actions of suramin and pentamidine are not fully known, but suramin appears to inhibit enzymes involved in NADH oxidation and pentamidine is a potent inhibitor of nucleic acid biosynthesis 55 ; . Eflornithine is a specific, irreversible inhibitor of ornithine decarboxylase, an enzyme required for parasite polyamine synthesis 148 ; . Current treatment.
