Cytotoxic drugs eg, cyclophosphamide ; , he developed a pharmacodynamic model relating the surviving white blood cell fraction to area under the concentration-time curve AUC ; . He found an inconsistent relationship between AUC and dose and was thus unable to develop a pharmacodynamic model using dose administered as a variable. In contrast to some other investigators who have suggested that dose alterations for doxorubicin are not necessary in patients with decreased hepatic function, he found that decreased hepatic function does influence AUC, suggesting that this modification issue should be explored further. He applied a similar pharmacodynamic methodology for etoposide, where protein binding is an important consideration, and developed a similar pharmacodynamic model using AUC. In his presentation, "Pharmacokinetics and Pharmacodynamics of Carboplatin and 5-Fluorouracil, " Alan Forrest, PharmD, from the Millard Fillmore Hospital first explained some of the difficulties in developing pharmacodynamic models for anticancer compounds, including the delayed or "hit-and-run" effect, when the effect you are modeling occurs some time after the drug has disappeared from the circulation. A good example is using the nadir white blood cell or platelet count, which typically occurs on days 12 to 16; however, many anticancer compounds can not be measured in the plasma at this time. He summarized the early work with carboplatin, perhaps the first anticancer agent for which a pharmacodynamic model was developed, and described the strategies used to develop the familiar relationship between renal function and platelet nadir for this agent. He reiterated that pharmacodynamic models work best when there is narrow interindividual variability. Dr Forrest then went on to describe his recent work with a 72-hour fluorouracil infusion modeling AUC vs leukopenia or stomatitis. In his unique model, he considered several covariate factors such as the presence of other drugs eg, dipyridamole ; . He concluded by indicating that future approaches may model the cost benefit to effect toxicity ratios to AUC. William Evans, PharmD, from the St Jude Children's Research Hospital closed the session with "Pharmacokinetics and Pharmacodynamics of Anticancer Agents: Overview and Future Considerations." Dr Evans opened with the basic premise that systemic exposure predicts response or toxicity better than the dose administered. He discussed the published work from his group supporting this concept for methotrexate and teniposide. He then gave details about the randomized trial in progress comparing prospective dose adjustments to optimize AUC for methotrexate, cytarabine, and teniposide compared to standardized doses for these agents in children with leukemia. Dr Evans then discussed the present focus of his work at St Jude looking at polymorphic drug metabolism. He has been using several high-powered molecular methods to examine the relationships between drug metabolism and response and or toxicity. He cautioned, though, that these molecular tests for drug-metabolizing enzymes become important only if there is an established relationship between drug concentration or AUC and response or toxicity. For example, these tests may be useful in determining compliance or might become especial15 and terfenadine.
