Sir: Somatization is a psychological process whereby psychological distress is expressed in the form of somatic symptoms.1 Conversion disorder is a type of somatization that presents with signs and symptoms limited to the central nervous system.2 Despite the substantial literature, the exact etiology of conversion disorder is still controversial. Two important DSMIV criteria for the diagnosis of conversion disorder include association of psychological factors with either initiation or exacerbation of symptoms and inability to explain the symptoms due to any general medical condition. Some earlier studies have postulated that the prognosis of this disorder varies with symptoms; for example, seizures and tremors would have worse prognosis as compared to weakness and sensory disturbance.3 We present an interesting case by its complicated course and sudden improvement of what we believe is conversion disorder. Case report. Mr. A, a 30-year-old male Honduran native, was brought to the emergency department by his family secondary to fainting, questionable seizures, fever, recent right arm numbness, and agitated behavior. The history was obtained from the patient's sister as Mr. A was unable to answer questions reliably and did not speak English. The sister reported that his symptoms began approximately 2 weeks earlier, when he experienced an episode of twitching and foaming at the mouth consistent with a seizure. He was seen by a medical doctor at another hospital at that time and was discharged home; however, 2 days later he experienced a second seizure with the same symptoms. Mr. A was again taken to the hospital, where he was admitted and discharged home 2 days later. At that time, Mr. A was started on phenytoin 100 mg t.i.d. He was also started on risperidone 1 mg nightly and diazepam 5 mg.
SIR: Cases of female anorgasmia caused by monoamine oxidase inhibitors MAOIs ; were reported in this journal in three separate letters published in 1983 1-3 a literature search revealed only one subsequent published paper 4 ; on this topic. In order to augment the sparse literature, ! would like to describe an additional case. Ms. A, a 36-year-old woman with a long history of obsessive-compulsive disorder, which had been refractory to drug although MAOIs were not tried ; , behavioral, and traditional psychotherapeutic approaches, was admitted to the inpatient psychiatric service following a suicide attempt with an overdose of imipramine. The patient's depression and obsessions, but not her compulsive handwashing, responded to phenelzine, 60 mg day. Three weeks after discharge after 5# weeks of taking.

Purpose: High epidermal growth factor receptor EGFR ; gene copy number is associated with poor prognosis in lung cancer, but such findings have not been reported for HNSCC. A better understanding of the EGFR pathway may improve the use of EGFR inhibitors in HNSCC. Patients and Methods: EGFR status was analyzed in 86 tumor samples from 82 HNSCC patients by fluorescent in situ hybridization FISH ; to determine EGFR gene copy number, by polymerase chain reaction and direct sequencing for activating mutations, and by DNA microarray and immunohistochemistry for RNA and protein expression. The results were associated with patient characteristics and clinical end points. Results: Forty-three 58% ; of 75 samples with FISH results demonstrated EGFR high polysomy and or gene amplification FISH positive ; . The FISH-positive group did not differ from the FISH-negative group with respect to age, sex, race, tumor grade, subsites and stage, or EGFR expression by analyses of RNA or protein. No activating EGFR mutations were found. However, the FISH-positive group was associated with worse progression-free and overall survival P .05 and P .01, respectively; log-rank test ; . When microarray data were interrogated using the FISH results as a supervising parameter, ECop which is known to coamplify with EGFR and regulate nuclear factor-kappa B transcriptional activity ; had higher expression in FISH-positive tumors. Conclusion: High EGFR gene copy number by FISH is frequent in HNSCC and is a poor prognostic indicator. Additional investigation is indicated to determine the biologic significance and implications for EGFR inhibitor therapies in HNSCC. Efficacy on femoral neck fractures has not been established Comparator Medications: Alendronate, risedronate sodium, raloxifene, disodium etidronate, calcitonin and teriparatide are included in the Scottish Intercollegiate Guidelines Network SIGN ; guideline, Management of Osteoporosis and the National Institute of Health and Clinical Excellence NICE ; appraisal, Bisphosphonates, selective oestrogen receptor modulators and parathyroid hormone for the secondary prevention of osteoporotic fragility fractures in postmenopausal women. Strontium ranelate was accepted for restricted use by the SMC in July 2005. Ibandronic acid Bonviva ; is accepted for use within NHS Scotland for the treatment of osteoporosis in postmenopausal women in order to reduce the risk of vertebral fractures. Ibandronic acid 150mg monthly is superior to daily ibandronic acid in terms of lumbar spine bone mineral density at 1 year. Compared with placebo, daily administration of ibandronic acid results in a relative risk reduction for vertebral fractures of 62%. Unlike some other bisphosphonates, efficacy in reducing femoral neck fractures and other non-vertebral fractures ; has not been established. Although its cost is similar to that of other bisphosphonates, ibandronic acid demonstrates more limited efficacy, and therefore may not provide such good value for money. Bisphosphonates act by reducing osteoclast-mediated bone resorption, which results in a decrease in bone turnover, an increase in bone mineral density BMD ; and a reduction in fracture risk. Trials comparing placebo, daily administration and an intermittent administration of ibandronic acid gave results of relative risk reduction for new morphometric vertebral fractures compared to placebo of 62% and 50% for the daily and intermittent groups respectively. The risk reduction for non-vertebral fractures compared with placebo was 69% and 37% for the daily and intermittent groups respectively. The second study results looked at % change from baseline in lumbar spine BMD for differing regimes of ibandronic acid. They showed increases in lumbar spine BMD at 1 year were 3.9%, 4.3%, 4.1% and 4.9% for the daily, 50mg monthly, 100mg monthly and 150mg monthly ibandronic acid groups respectively. Two year findings for this study reported increases in lumbar spine BMD of 6.6% and 5.0% for the ibandronic acid 150mg monthly and 2.5mg daily groups respectively. The incidence of drug related and or unrelated adverse events, as well as rates of adverse events resulting in withdrawal from the study, were similar for the daily and monthly arms. There are no comparative trials of ibandronic acid with other standard treatments for osteoporosis in postmenopausal women. To study the treatment of osteoporosis this is defined as T-score at hip or spine of -2.5 but the initial trial the T-score was between -2 and -5. Studies have proved that changes from daily to weekly dosing improves compliance and efficacy of bisphosphonates but this is not clear in relation to monthly dosing. There is evidence in relation to patient preference but as patients with poor compliance where excluded from study 2 it is difficult to the numbers of non-compliant patients in each arm. Assuming equivalent clinical effectiveness the manufacturer used a cost minimisation analysis showing a cost saving with the use of ibandronic acid. Equivalence in shown in preventing vertebral fractures but there are different effects on femoral neck fractures. Budget impact indicates a gross direct drug cost of 35, 000 for Fife in year 1 rising to 550, 000 by year 5. This implies near market dominance but this is unlikely due to similar price with competitors. Expenditure in Primary care in October 2005 was 772 16 items ; . Do not add to the formulary. Absence of data relating to hip fracture risk reduction. Of benefit to those patients were side effects are problematic and are unkeen for weekly preparation.
