On July 30, 1992 the worker undergoes a psychological assessment by Dr. Alvaro, psychologist. The worker described continued back and leg pain "originating in the left shoulder and radiating down the entire length of his back, through the buttocks, right leg and left leg to the foot". The worker said he experienced most pain upon waking and when sitting for any length of time. "At present his strategy for pain management consists of inactivity i.e. rest ; and medication usage considers himself markedly restricted". Dr. Alvaro recommends the worker be referred for psychotherapeutic treatment focusing on depression, pain management and agitation. Dr. Doyle's progress reports through July, August and September note no significant change in the worker's reports of pain. Note is made of counselling the worker on medication usage with respect to problems with tolerance and dependency. The worker is examined by Dr. Preto, orthopaedic surgeon, on September 29, 1992.
